Description:Harbor's Edge is a premier resort-style Continuing
Care Retirement Community (CCRC) located on the harbor in Norfolk,
Virginia. Harbor's Edge has a $225 million expansion underway, the
River Tower, that includes an additional 24-story tower, doubling
the square footage and adding 147 residential living units and 20
assisted living units. When complete, Harbor's Edge will have 300
residential units in addition to a 33 bed (Medicare/Private Pay)
Skilled Nursing unit and 80 Assisted Living/Memory Care units. The
Community will employ approximately 300 employees.The LPN
Supervisor of Assisted Living and Memory Support (AL/MS) is
responsible for overseeing the Assisted Living and Memory Support
departments under the direction of the MS Manager and Health
Services Administrator. The Supervisor is expected to provide
resident care in accordance with current Federal, State, and local
standards, guidelines and regulations. The Supervisor enforces
policies and procedures, assists with staffing plans, reports and
budgets.The ultimate goal of quality resident care is to enable
each resident, where possible, to return to the highest level of
functioning, to attain a new stage of independence, or to at least
prevent regression. Resident care is not passive maintenance; an
active approach will always be taken.Responsibilities
- Supervise the activities of the Assisted Living and Memory
Support departments in compliance with all applicable rules,
regulations, and guidelines. Make reports and recommendations to
the Administrator as necessary or required concerning the operation
of the departments. Must be able to communicate effectively with
family members, residents, and physicians in a timely manner.
- Provide daily supervision of staff to include accountability
for making recommendations on hiring, terminations, disciplinary
action, training and promotions. Create and assign work schedules
and ensure appropriate staffing levels. Orient and train new
employees.
- Oversee development of and compliance with resident care plans,
providing direct care to residents as needed. Observe residents for
change in condition and report any changes to Administrator and
resident's physician, as directed. Document all pertinent/relevant
observations, communication with the resident's physician &
responsible party, clinical procedures and daily assistance/care by
the end of each shift. Handle routine problems and concerns of
residents, escalating serious issues to Administrator for
review/action plan.
- Ensure all protocols and procedures are followed. Monitor
ongoing operations and participate in assessing compliance with
established QA standards. Identify staff educational needs and
coordinate training with Administrator.
- Perform resident assessments as required by regulations and
facility policy. Participate in evaluation of new residents and
level of care transitions.
- Develop methods for coordination of the departments' services
with other resident services, to ensure the continuity of the
residents' total regimen of care. Attend scheduled
interdisciplinary resident care meetings.
- Assist with departmental planning and budgets. Arrange for
appropriate supplies and equipment, maintaining compliance with
departmental budget.
- Perform other duties as assigned. Attend in-service classes and
staff meetings. Assist in developing/updating written policies and
procedures and job descriptions.Requirements:Qualifications-
- High school diploma/GED at a minimum
- Current state license as an LPN in the Commonwealth of
Virginia
- At least one year of experience in an Assisted Living/Memory
Support facility
- At least one year of experience at this level of
responsibility
- CPR and First Aid Trained with a valid certificate/card
- VAI/ISP Certification
- Must be physically able to lift up to 25 pounds
- Must be capable of sitting, standing, walking, climbing stairs,
reaching, turning, bending, stooping, crouching and kneeling,
balancing and maintaining body equilibrium to prevent falling,
reaching over the head, pushing/pulling, and grasping with both
hands
- Excellent verbal and written skills
- Able to follow spoken and written instructions
- Desire to work in a Continuing Care Retirement Community
(CCRC)
- Must be able to deal tactfully with personnel, residents,
family members, visitors, and the general public
- Must be able to pass a criminal, sex offender, PPD, and drug
