You will help develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including at time of terminal illnesses.Job Requirements:

  • 4-year degree in Health and Human Services
  • 1 year of Long Term Care experience required
  • Case Management/ Care Coordination experience strongly preferred
  • Experience with Community Health Resources strongly preferred
  • Experience initiating care plans and both subjectively and objectively conducting assessments
