At MEDIKO, our people are our most important resource and the positions we bring on will support our company's rapid growth.Please visit our website at www.MEDIKO.com to learn more about the company!MEDIKO is HIRING the following position for our new partner, the Norfolk City Jail:Position, LPC/LCSW - Discharge PlannerPay, Starting @ $65,000/yearSchedule/Shift, FT, M-F, 8hr shiftsWe are looking for LPC/LCSW - Discharge Planner for our Norfolk City Jail to join a diverse, fun day shift team who bring knowledge and skills from a variety of backgrounds.What You'll Do:Monitors chronic care logs and caseload as needed to supplement referrals by staff.Develops discharge planning referral criteria for intake medical screeners.Consults routinely with medical providers on current caseload to identify patients meeting criteria for receiving pre-release health planningConsults with the MEDIKO behavioral health provider (including psychiatrist and licensed mental health professionals) on specific patients with complex needs that require supplemental coordination in addition to individual treatment plans or behavioral health referral plans upon release from custodyParticipates in multidisciplinary planning in support specialized transitional programs dedicated to seriously or chronically ill patients (such as community based mental health teams and HIV Jail Linkage programs)In special cases may support patients by providing required applications and assistance in confirming eligibility, as well as by referring them to assigned agency and correctional facility program staffTo the extent possible and permitted by notice of release, make appointments for patient with willing community resources/health providers by:Establishing lines of communications to streamline appointment schedulingPromoting a common understanding of admission criteria, eligibility and enrollment requirements when applicableRespond to requests from Security Staff as well as MEDIKO Staff in accordance with MEDIKO and Facility Guidelines. This includes following the rules and regulations for employee conduct/behavior and chain of command set forth by the command staff at all times while working within the detention/correctional facility.Perform additional duties as assigned.What We Require:LPC/LCSW RequiredMinimum of two years experience, preferably in correctional health or criminal justice environment as case manager; discharge planner; nurse performing similar tasks related to referral, case management, and/or care coordination; or mental health related service coordinatorBachelors Degree in social sciences, human services, or health care related field or Nursing DegreeExcellent written and verbal communication skills; excellent computer skillsFamiliar with health records, medical terminology, and health information servicesMust receive and maintain satisfactory background investigation reportsClinical credentials such as nurse, social worker, screener, or case manager including training, licensure, and certification is desirable but not a requirement.Other relevant professional experiences or educational backgrounds will be considered as deemed appropriate.
