Job Description:

ChenMed, a leader in healthcare management and services, specifically catering to seniors with complex health issues, is seeking an experienced and compassionate Nurse Practitioner to join our team. This role is pivotal in offering a blend of preventative, routine, and acute care to our patients, ensuring the highest quality of life through personalized treatment plans. As part of our dedicated team, you will have the opportunity to collaborate with physicians, specialists, and other healthcare professionals to deliver comprehensive care tailored to the unique needs of each patient. Conduct comprehensive patient assessments to establish accurate diagnoses and manage complex health issues. Develop and implement treatment plans in collaboration with healthcare team members. Prescribe medications and therapies based on patient-specific needs. Monitor patient outcomes to adjust treatment plans as necessary. Educate patients and their families on preventive care and disease management. Stay current with advancements in the field to ensure the application of up-to-date practices. Participate in multidisciplinary team meetings to coordinate care and optimize patient outcomes. Strong diagnostic skills Proficiency in electronic health record (EHR) software Excellent communication and interpersonal skills Ability to manage time and prioritize tasks efficiently Experience in geriatric care or chronic disease management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license Certified Nurse Practitioner (NP) license in the state of practice Basic Life Support (BLS) Certification Advanced Cardiac Life Support (ACLS) Certification preferred
