Job Description IDR Healthcare is seeking a travel nurse RN Hospice for a travel nursing job in Colonial Heights, Virginia. Job Description & Requirements - Specialty: Hospice - Discipline: RN - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Hospice RN/nColonial Springs, VA /nSurrounding Areas (Chesterfield, Tri-Cities, Hopewell, Petersburg, Colonial Heights) /n /nPay: $50/hr/n Primary Responsibilities: /n/n /n/t - /n/t Conduct comprehensive assessments of high-risk members to evaluate clinical, functional, and psychosocial needs. /n/t/n/t - /n/t Develop and implement individualized, goal-oriented care plans in collaboration with primary care providers and the interdisciplinary care team. /n/t/n/t - /n/t Continuously monitor, evaluate, and adjust care plans to ensure appropriate, cost-effective levels of care and optimal patient outcomes. /n/t/n/t - /n/t Identify and coordinate non-medical support services (e.g., housing, transportation, and community resources) to enhance compliance with treatment plans. /n/t/n/t - /n/t Facilitate seamless transitions of care by collaborating with physicians, social workers, discharge planners, and claims professionals. /n/t/n/t - /n/t Utilize specialty resources and clinical expertise to achieve optimal patient outcomes and minimize preventable complications. /n/t/n/t - /n/t Maintain detailed documentation of assessments, care coordination activities, and measurable outcomes in compliance with organizational and regulatory standards. /n/t/n/t - /n/t Promote health education, disease prevention, and early intervention strategies to improve overall member well-being. /n/t/n/t - /n/t Serve as a patient advocate by ensuring privacy, confidentiality, and respect throughout all phases of the care process. /n/t/n /n/n Qualifications: /n/n /n/t - /n/t Associate’s or Bachelor’s degree in Nursing or a related healthcare field (BSN preferred). /n/t/n/t - /n/t Current, unrestricted state Registered Nurse (RN) license. /n/t/n/t - /n/t Strong analytical and clinical judgment skills with the ability to interpret complex medical data. /n/t/n/t - /n/t Proven ability to work collaboratively across multidisciplinary teams and manage multiple priorities effectively. /n/t/n /n IDR Job ID 471899. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
