Description:Join the Five Star Home Health Care team as our new
Speech Language Pathologist!The Speech Language Pathologist
primarily responsible for continuous review of all aspects of
his/her caseload to include: appropriate utilization of services;
ensuring continued skilled need; monitoring of homeboundstatus;
review of documentationin the medical record; maintenance of
interdisciplinary communication and discharge planning.Job
Functions:Develops a care plan, which establishes goals based on
nursing diagnosis.Utilize med/surg skills such as phlebotomy, wound
care, and teaching for chronic diseases.Supervise, teach and
provide clinical direction to LPN and CNA's.Position: PRNSLP
Rates:OASIS SOC $135 per visitAssessment $105 per visitRoutine
Visit $95 per visitRequirements:Qualifications:Valid Virginia
Speech Language Pathologist licensePrior experience in home health
is preferred
